Polymorphism provides software that may facilitate introductions between you and licensed care providers. Using the platform does not, by itself, establish a clinician–patient relationship with Polymorphism.
What you consent to
- Sharing accurate information so agencies can assess fit and availability.
- Electronic communications regarding scheduling, intake, and account administration.
- Recording audit logs of certain actions for compliance and dispute resolution.
What requires separate consent
Clinical treatment, medication decisions, and hands-on care require agreements directly with the responsible licensed professionals and organizations. Follow their instructions for emergencies.
